Patient Care
- Participate in ongoing evaluation of a patient’s functional status—including muscle function, endurance, coordination, communication abilities, self-care skills, and work capacity—as delegated by the Occupational Therapist.
- Assist in assessing the home environment for safety hazards or barriers to independence.
- Teach and retrain patients in daily activities lost due to illness or injury, in alignment with organization policy.
- Follow the established treatment plan and goals created by the Occupational Therapist; document patient response and progress.
- Identify patient or caregiver needs for additional home care services; consult with the supervising OT and assist with referrals as appropriate.
- Participate in educating caregivers and interdisciplinary team members on patient treatment regimens as delegated.
Communication & Documentation
- Maintain appropriate clinical records and promptly report any changes in patient condition to the supervising OT.
- Submit accurate, timely documentation according to organizational policy.
- Communicate effectively with all personnel involved in the patient’s care to ensure coordinated and efficient service delivery; document all relevant communication per policy.
- Ensure the clinical record reflects ongoing communication with the OT, patient condition changes, treatment responses, plan-of-care updates, and patient outcomes.
Additional Responsibilities
- Attend and contribute to in-services, interdisciplinary group meetings, and other required meetings to support coordinated care.
- Receive supervisory visits from the Registered Occupational Therapist at least every 30 days.
- Actively participate in quality assessment and performance improvement activities.
- Perform additional duties as assigned.
Qualifications
- Graduate of an AOTA-accredited Occupational Therapy Assistant program.
- Current state certification or licensure as required.
- Minimum of one year of clinical experience as a COTA.
- Current CPR certification.
- Licensed driver with a reliable, insured automobile meeting organizational requirements.
