Responsibilities
- Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
- Assists residents with or performs activities of daily living in accordance with care plans and established policies and procedures.
- Assists residents with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
- Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake. Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
- Assists with weighing residents according to facility policy and records weight in designated locations. Reports weight changes to nurse and supervisor.
- Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown, or skin tears to nurse and supervisor.
- Assists nursing staff in carrying out toileting program activities.
- Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
- Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (e.g., resident refusal, reassignment to nursing unit).
- Reports changes in a resident’s condition to the resident’s nurse and supervisor.
Requirements
- Certified Nursing Aide in good standing with the certification board or a graduate of a state-approved nursing assistant training program who has passed a state-approved competency examination (written and skills) within 120 days of employment REQUIRED.
- High School Graduate/GED preferred.
- Current CPR/BLS certification preferred.
- At least one year of experience as a certified nursing assistant in long-term care preferred.
- Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
