About the Role

Provides routine daily nursing care and services to assigned patients in accordance with the patient’s assessment and care plan, and as directed by the nurse supervisor.

Responsibilities

  • Provides care ensuring patient/resident safety.
  • Applies and releases restraints, and provides exercise.
  • Assists residents in and out of bed and into wheelchair using transfer belts and proper body mechanics.
  • Assists residents with bathing, including bed baths, tub baths, and showers.
  • Assists residents in dressing, undressing, and personal grooming (oral/denture care, hair brushing, nail trimming, skin care, and shaving).
  • Assists residents to and from activities as requested.
  • Assists residents to the dining area for meals; passes food trays according to diet cards and needs; documents nourishment intake and identifies the four basic food groups.
  • Assists with admissions, discharges, and transfers; understands and utilizes care plans.
  • Cleans bedpans, empties catheter bags, provides catheter care, and assists with incontinent care.
  • Offers fluids at appropriate times, distributes fresh water, and picks up used water pitchers.
  • Properly documents care in accordance with established guidelines; reports on patient status and notifies nurse of any changes.
  • Reports skin irritation or problems with ostomy bags to the Charge Nurse.
  • Provides range-of-motion exercises for residents.
  • Provides routine care for bedfast patients, including intermittent turning for bedridden residents.
  • Recognizes and reports signs/symptoms of abuse to the Charge Nurse/Administrator; identifies and reports changes in condition.
  • Replaces bedding with clean linens per schedule or as needed.
  • Takes and records vital signs (temperature, blood pressure, respiration) and documents health changes, I&O, and care given.

  • Participates in center/agency surveys (Licensure/JCAHO) and required reports.
  • Attends continuing education programs to stay updated in the field.
  • Attends mandatory in-services.
  • Honors patients’ rights to fair treatment, self-determination, privacy, and property.
  • Complies with corporate compliance programs and safety regulations (e.g., infection control, fire protection).
  • Follows safety procedures when performing tasks or working with equipment.
  • Performs other related duties as directed by the nurse supervisor.

Requirements

  • Some high school education or a high school diploma/GED required.
  • On-the-job training provided.
  • Must not have adverse findings on criminal background checks.
  • Must demonstrate competency to provide nursing services consistent with CMS Waiver requirements.

Preferred Qualifications

  • High school graduate preferred.
  • Previous experience preferred.
  • Familiarity and willingness to work with the elderly population.

Location: Georgia-Baldwin. Schedule: Full-time, variable shifts.
