Responsible For
- Provides emotional and physical support to terminally ill individuals and their families/significant others.
- Aids in improving the quality of life for terminally ill individuals during the final stage of life.
- Offers respite care to families and significant others caring for terminally ill individuals at home.
- Maintains and promotes the independence of the patient and family, ensuring a safe, healthy environment.
- Participates in Interdisciplinary Group meetings as needed.
Essential Duties
- Assists with personal care activities such as baths, mouth care, skin care, foot care, grooming, transferring, dressing, undressing, ambulation, turning, and positioning.
- Assists with household tasks including meal preparation, feeding, light housekeeping, changing linens, making beds, laundering clothing and bedding.
- Measures and records vital signs (temperature, pulse, respiration, blood pressure) within training limitations.
- Ensures patient safety by performing safe use of equipment and assisting with therapy services as directed.
- Assists with medication administration and performs or assists with other tasks as per the personal services care plan.
- Reports any changes in patient conditions or incidents immediately to the Primary Care Nurse/Director of Hospice Services.
- Accurately documents services provided and submits documentation in a timely manner.
- Participates in staff development programs and accepts responsibility for ongoing skill maintenance and adherence to Hospice policies and procedures.
Skills And Abilities
- Able to function independently with minimal supervision, maintaining a professional relationship with patients and families.
- Compliant with health clearance and reference provisions of Hospice's conditions of employment.
- Capable of transporting oneself to patient's home/Facility, potentially visiting multiple locations in a workday.
Qualifications
- Completion of a state-recognized and federally compliant training program and/or competency evaluation.
- At least six months of experience as a Certified Nursing Assistant.
- Compliance with in-service regulations and requirements.
