Responsible For
- Provides emotional and physical support to terminally ill individuals and their families/significant others.
- Aids in improving the quality of life for terminally ill individuals during the final stage of life.
- Offers respite care to families and significant others caring for terminally ill individuals in the home.
- Maintains and promotes the independence of the patient and family, ensuring a safe, healthful environment.
- Participates in Interdisciplinary Group meetings as needed.
- Assists with personal care activities:
- Baths (tub, showers, bed, bath)
- Mouth care, denture care, skin care, foot care
- Cleaning room (shampoo, dry and comb hair; shave, ordinary care of nails)
- Transferring (bed to chair, to wheelchair, to tub, to shower)
- Dressing and undressing activities
- Ambulation
- Turning patient in bed and positioning for comfort
- Assists with household tasks directly essential to the patient's health:
- Planning and preparing meals
- Assisting in feeding patient if appropriate
- Lighting housekeeping of patient's immediate environment
- Changing linen, making bed
- Laundering of patient's clothing and bedding
- Maintains and records vital signs (oral, rectal, axillary temperatures, pulse, respiration and blood pressure) within training limitations.
- Ensures safety needs of patients, including proper use of equipment (foot stools, side rails, step stools, wheelchairs, oxygen, etc.).
- Performs range of motion and other simple procedures as an extension of therapy services within training limitations.
- Aids in self-administration of medication reminders and assists with activities as per the personal services care plan.
- Documents services provided accurately and submits documentation in a timely manner.
- Participates in staff development programs as appropriate.
- Maintains skills, learns, and adheres to Hospice policies and procedures.
- Promotes the image and reputation of the System through servant leadership and open communication.
Qualifications
- Completion of state recognized and federally compliant training program and/or competency evaluation.
- At least six-months experience as a Certified Nursing Assistant.
- Compliant with in-service regulations and requirements.
