Conditions of Employment
- Prior training and experience as a Home Health Aide with Indiana certification as a Nurse Aide;
- Must pass Drug Screen and Background check;
- Compliance to Annual Flu Shot and Covid Vaccination Policy or ability to provide exemption documentation;
- Pass TB test;
- Provide an active auto insurance policy as a licensed driver in the State of Indiana with the proper level of coverage as directed;
- Provide an active driver’s license;
- Ability to provide proof of eligibility to work in the United States;
- High level of integrity, ethics and professionalism.
Responsibilities
Travel to where Patient lives and calls home.
Provides care and services to patients according to the care plan or as approved by an RN.
Assists with other activities of daily living as directed by an RN.
Offers support and encouragement to patients and families.
Functions within acceptable time frames for daily patient visits.
Clearly communicates information regarding changes in the patient’s mental/physical condition, complaints, or special requests to the Agency nurse, and ECF unit manager if applicable.
Demonstrates good judgment in knowing what information needs to be communicated.
Handles communication in a timely manner.
Maintains documentation of patient care provided on both Agency forms, and the ECF record if applicable.
Uses appropriate Mobile Care documentation module or paper forms.
Records information accurately, thoroughly, and in a timely manner
Committed to Agency resolution against fraud and abuse.
Knowledgeable in hospice philosophy and state and federal rules and regulations.
Respects confidentiality of patients/families and other Agency employees.
Successful Capabilities
- Must be able to read and write English proficiently;
- Must be state tested nursing assistant in good standing;
- Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.
