Certified Nursing Assistant (CNA)

About the role

We are seeking a dedicated, compassionate, and experienced full-time Certified Nursing Assistant to join our team serving hospice patients in and around Albuquerque, NM, specifically the North East Heights/East Mountains region. The CNA is a paraprofessional member of the interdisciplinary group who works under the supervision of a Registered Nurse, performing various services to meet patients' personal needs and promote comfort. The role involves observing patients, reporting and documenting observations, and ensuring quality, continuity, and safety of care.

Responsibilities

• Perform personal care tasks after receiving patient assignments and attend to requests promptly; report any inability to perform tasks to the Registered Nurse Case Manager immediately.
• Ensure patient safety and use equipment (foot stools, side rails, oxygen, etc.) safely and properly.
• Conduct vitals and assist with Activities of Daily Living (ADLs), including but not limited to:
  • Personal care (baths, oral hygiene, shampoos, changing bed linen).
  • Assisting in dressing and undressing patients.
  • Assisting in feeding patients as assigned.
• Provide proper care and observation of patient’s skin to prevent tissue breakdown over bony prominences.
• Report patient conditions and significant changes to the Registered Nurse Case Manager; be aware of caregiver or other individuals living with the patient and interpersonal issues.
• Assist in ambulation and exercise as instructed by the hospice nurse or therapist.
• Assist with bedpans, urinals, and light laundry needs as assigned.
• Perform range of motion and other simple procedures as an extension of therapy services, as ordered, with demonstrated skills competency.
• Provide respite for patient/family/caregiver when on-site, as appropriate.
• Maintain patient’s immediate living space tidy, as assigned.
• Adhere to the organization’s documentation and care procedures and standards of personal and professional conduct.
• Participate in quality assessment performance improvement teams and activities.
• Respect patient and family/caregiver environment and personal needs.
• Participate in on-call duties as assigned.
• Meet the training requirements of the State.

Requirements

• Active CNA license in the state you are applying and will be working.
• One year of Certified Nursing Assistant experience required.
• One year of home health/hospice experience preferred.
• At least 18 years of age.
• Ability to read and follow written instructions and document care given.
• Self-directing with the ability to work with little direct supervision; provides a calm manner when in a patient’s home.
• Empathy for the needs of the ill, injured, frail, and impaired.
• Possess and maintain current CPR certification.
• Demonstrate tact, patience, and good personal hygiene.
• Licensed driver with an automobile that is insured in accordance with organization requirements and in good working order.

Job Limitations

The Certified Nursing Assistant will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act.
