Responsibilities

• Provide personalized care and support to patients in their homes, including assistance with daily living activities, personal care, and mobility
• Monitor and report patient condition to nurse supervisors and other healthcare professionals as needed
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Maintain accurate and detailed records of patient care and services provided
• Promote a safe and healthy environment for patients and staff
• Participate in training and professional development opportunities to enhance skills and knowledge

Requirements

• Certification as a Certified Nursing Assistant (CNA) in the State of Texas
• High school diploma or equivalent required; nursing education preferred
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
