Certified Nursing Assistant CNA

About the Role

As a Certified Nursing Assistant on our evening shift, you will be responsible for providing direct patient care, including vital sign monitoring, wound care, and assistance with daily living activities. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident. Your attention to detail, organizational skills, and ability to work effectively in a fast-paced environment will be essential in ensuring the highest quality of care.

Responsibilities

• Provide direct patient care, including vital sign monitoring, wound care, and assistance with daily living activities
• Develop and implement individualized care plans in collaboration with the Interdisciplinary Care Team
• Assist with admissions, transfers, and discharges, as needed
• Communicate effectively with residents, families, and healthcare team members
• Maintain accurate and up-to-date records of patient care and progress
• Participate in quality improvement initiatives and contribute to a culture of excellence

Requirements

• Current CNA certification in the state of Utah
• High school diploma or equivalent required
• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
• Excellent communication and interpersonal skills
• Physical stamina to perform tasks that require standing, walking, and lifting
