About The Role

As a CNA, you'll play a vital role in supporting our patients' quality of life through consistent, one-on-one, relationship-based care in their homes. Key Responsibilities
- Assist with activities of daily living (ADLs) (bathing, dressing, grooming, toileting, mobility)
- Provide companionship, emotional support, and social engagement
- Support meal preparation, nutrition support and light housekeeping
- Monitor and report changes in condition
- Follow care plans and document accurately using EMR standards
Qualifications
- High school diploma or GED
- Active CNA certification
- Iowa CPR certified
- Valid driver's license and reliable transportation
- Ability to work independently in a non-facility setting
- Home health, long-term care, or hospice experience preferred
