About the Role
CommWell Health is seeking a Certified Nursing Assistant (CNA) to join our team. This position will deliver compassionate, high-quality care to patients across the lifespan, partner with providers and care teams to ensure seamless, coordinated treatment, support patients and families in understanding health needs and next steps, document care accurately using our EHR system, participate in care huddles and team-based quality improvement initiatives, and serve as a key member of our Patient-Centered Health Home model.
Responsibilities
- Deliver compassionate, high-quality care to patients across the lifespan
- Partner with providers and care teams to ensure seamless, coordinated treatment
- Serve as a key member of our Patient-Centered Health Home model
- Document care accurately using our EHR system
- Participate in care huddles and team-based quality improvement initiatives
Requirements
- CNA certification, active and in good standing with the State of North Carolina
- HS Diploma/GED
- Current CPR/BLS certification
- Valid Driver’s License
Preferred
- One (1) year of clinical experience in a primary care, community health, or outpatient setting
- Experience with underserved or rural populations
- Familiarity with team-based care or PCMH/PCHH models
- Bilingual
