About the role
Provides compassionate care to patients and families in their homes. Works closely with interdisciplinary teams to coordinate care plans.
Responsibilities
- Assesses patient needs and develops personalized care plans
- Administers medications and treatments as prescribed
- Makes home visits to provide support and education to patients and families
- Collaborates with healthcare providers to ensure continuity of care
- Monitors patient progress and reports changes to healthcare team
Requirements
- Bachelor's degree in a healthcare-related field
- Current state licensure as a hospice aide or equivalent
- Experience in home health care or related field preferred
- Strong communication and interpersonal skills
- Ability to work flexible hours including evenings and weekends
Qualifications
- Excellent problem-solving and critical thinking skills
- Passion for providing compassionate care
- Ability to handle sensitive and confidential information
Skills
- Basic computer literacy
- Proficiency in Microsoft Office Suite
