Responsibilities
- Fulfills the resident's/patient's plan of care to improve or maintain their abilities.
- Notifies nursing supervisor of any changes in abilities, overall condition, or symptoms affecting care delivery.
- Performs nursing rehab techniques as instructed and approved by the community.
- Aids residents/patients with meals and nourishments, monitors food and fluid intake, and addresses dietary concerns.
- Assists residents/patients with transportation to activities and appointments, ensuring safety and timely arrival.
- Routinely rounds on the healthcare unit to assess residents/patients' needs.
- Promptly responds to calls for assistance and identifies when clothing and personal items need replacement or repair.
- Takes accurate vital signs and weights, notifying the nursing supervisor of any changes.
- Records accurate and legible information about resident/patient care and condition in the appropriate sections of the record.
- Maintains effective communication with the nursing supervisor regarding the residents'/patients' status.
- Attends staff meetings and educational offerings, completing assigned education by the stated deadlines.
- Applies educational content to practice and maintains attendance as per HR policy.
- Floors throughout the healthcare unit to meet the needs of residents/patients and promote teamwork.
- Utilizes supplies in a fiscally responsible manner.
Requirements
- High school diploma or GED.
- Current Certified Nursing Assistant (CNA) license.
