About the role

Location: Delaware/Lewis Center/Westerville, OH, and surrounding areas.

Provide compassionate, patient-centered personal care to hospice patients in their homes or facility settings, offering emotional support, companionship, and assistance with daily living activities as part of an interdisciplinary team.

Responsibilities

• Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.
• Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner.
• Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.
• Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.
• Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.
• Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.
