Responsibilities

  • Receive and follow written/verbal care assignments, providing daily care for residents, including:
    • Assisting with bathing, toileting, oral hygiene, skin care, grooming, nail care, and dressing.
    • Feeding residents, providing bedside water, snacks, and transferring residents to/from dining areas in compliance with nutritional plans.
    • Checking and providing care for catheters as assigned by a nurse.
    • Assisting residents with turning, positioning, exercising, transferring, and ambulation per facility protocol.
    • Answering call lights and responding to resident requests promptly.
    • Maintaining resident rooms, including making beds, cleaning bedside tables, and organizing personal belongings.
  • Observe and report changes in residents’ appetite, skin condition, or behavior according to facility policy.
  • Offer emotional support and actively listen to residents to build trust and promote independence.
  • Maintain confidentiality of all residents and their care.
  • Report and document incidents, accidents, complaints, or allegations using the designated occurrence reporting system.
  • Provide medical record documentation, including:
    • Taking and monitoring vital signs, intake/outputs, weights, and meal consumption.
    • Complying with documentation requirements for functional maintenance plans, 24-hour reports, CNA flow sheets, and other records.
  • Comply with safety and infection control measures, including:
    • Using protective devices (e.g., side rails, Geri chairs) under nurse direction.
    • Following emergency protocols for codes, fire safety, missing residents, and alarms.
    • Reporting abuse immediately and following universal precautions.
    • Maintaining a clean work environment and equipment.
  • Perform other duties as assigned by staff nurses.

Requirements

  • A high school diploma or GED (unless working under a student permit).
  • Certified by the Indiana State Department of Health (ISDH) as a C.N.A. in good standing. CNAs transferring from another state or graduating students may work for 120 days while awaiting Indiana certification.
  • Basic computer skills, including knowledge of Excel, Word, and other programs.
  • Thorough understanding of safe and effective nursing practices within Indiana.
  • Interpersonal skills, including:
    • Assisting licensed nurses with procedures within the scope of practice for nurse aides.
    • Ability to follow written/verbal instructions and make independent decisions when necessary.
    • Personal integrity and tact when dealing with residents, families, visitors, and agencies.
    • Ability to work harmoniously with professional and non-professional personnel.
    • Judgment in prioritizing tasks and adhering to policies/procedures.
    • Discretion in protecting confidential information and complying with HIPAA.
    • Patience, enthusiasm, and a willingness to support residents at their current functional level.
    • Ability to adhere to a professional code of ethics.
    • Ability to communicate resident conditions to appropriate staff.
