Hours: Monday through Friday, 08:30 AM – 5:00 PM
Responsibilities
- Provides or assists in activities of daily living (ADLs): personal hygiene, grooming, mouth, skin care, and nails.
- Assists with toileting, positioning, transfer activities, frequent turning and positioning of bed-bound residents/patients.
- Assists with prescribed exercise and range-of-motion (ROM) activities taught by the Hospice Nurse.
- Performs light housekeeping of the individual’s personal unit or room, including changing bed linens and patient laundry as needed.
- Feeds and assists with meals.
- Obtains temperature, pulse, and respirations under the supervision of the Hospice Nurse.
- Reports any changes in the resident’s/patient’s condition to the Hospice Nurse and, if in a facility, to the facility nurse; documents the information on the Home Health Aide Certification of House form, including date, time, and the person receiving the report.
- Assists or reminds the patient to take medication in compliance with regulatory and accrediting body regulations.
Requirements
- High School Diploma or equivalent.
- State Certified Nursing Assistant (CNA) license.
- Experience in hospice or home health preferred; no experience necessary, but 1 year is preferred.
- Willingness to travel.
- Valid driver’s license and reliable transportation.
