Schedule

Full-time, Monday–Friday, 3 PM–11 PM

Requirements

  • Licensed Nursing Assistant in the state of practice (CNA)
  • CPR/BLS Certified
  • Prior experience working in an AL/LTC/SNF facility or Home Health (REQUIRED)
  • Enjoy working with Senior Citizens; Positive Attitude; Professional, Responsible & Dependable
  • Must be willing to travel
  • Valid driver’s license and auto insurance coverage with reliable transportation
  • Hospice experience preferred

Responsibilities

  • Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care
  • Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner
  • Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life
  • Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments
  • Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care
  • Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts
  • Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse
  • Accurately document care provided in accordance with Care 360 Hospice policies and procedures
  • Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences
  • Provide support and education to family members and caregivers within the scope of the CNA role
  • Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance
  • All other duties as assigned
