Position Summary
The Certified Nursing Assistant (CNA) works in collaboration with the Program Registered Nurse (RN), Program Director, Resident Manager, and site supervisors to support and coordinate healthcare services for program consumers. The CNA serves as a liaison between consumers, healthcare providers, hospitals, and program staff to promote continuity of care, compliance with medical services, and positive health outcomes. This position assists consumers in accessing healthcare services, supports medical appointment coordination, maintains healthcare documentation, participates in interdisciplinary treatment planning, and promotes consumer wellness through education and daily health-related supports.
Essential Duties and Responsibilities
- Coordinate and schedule initial, follow-up, annual, and specialty medical appointments for consumers.
- Prepare and complete appointment-related paperwork and documentation.
- Attend medical appointments as necessary or provide education to staff regarding the purpose of appointments and treatment recommendations.
- Clarify physician orders and communicate recommendations to appropriate staff members.
- Track appointments and notify the Program RN or Assistant Program Director of consumer refusals, missed appointments, or barriers to receiving care.
- Maintain systems to ensure continuity of care and appropriate healthcare follow-up.
- Communicate with hospital case management staff within 24 hours of a consumer's hospital admission.
- Facilitate information sharing between hospital staff, program staff, and treatment providers.
- Participate in treatment team meetings and discharge planning meetings as appropriate.
- Maintain regular contact with hospitalized consumers through phone and in-person visits when required.
- Care for post-discharge follow-up care and ensure discharge recommendations are implemented.
- Aid consumers in understanding the importance of personal healthcare and independent activities of daily living (IADLs).
- Support staff and consumers in maintaining healthy lifestyles and wellness practices.
- Help ensure nutritious meals are planned and prepared daily in accordance with consumer needs.
- Promote preventive healthcare and encourage consumer participation in medical treatment and follow-up care.
- Maintain accurate healthcare documentation in the Electronic Health Record (EHR).
- Review consumer medical records and medical books on a quarterly basis to ensure compliance with applicable regulations and agency requirements.
- Identify and report documentation deficiencies or healthcare compliance concerns to the Program RN, Unit Director, or Assistant Program Director.
- Aid in the development, implementation, education, and monitoring of healthcare protocols and procedures.
- Support efforts to obtain and maintain documentation related to Durable Power of Attorney for Healthcare and Advance Directives, when appropriate.
- Participate as an active member of the multidisciplinary treatment team.
- Attend case conferences, team meetings, and trainings as required.
- Provide relevant healthcare information to support consumer treatment planning and service coordination.
- Collaborate with healthcare providers, direct care staff, family members, and community partners to promote quality care and consumer well-being.
Qualifications
- Current Certified Nursing Assistant (CNA) certification.
- Valid driver's license with an acceptable driving record.
- Strong verbal, written, and interpersonal communication skills.
- Proficiency with computer applications and electronic documentation systems.
- Ability to work effectively as part of a multidisciplinary team.

- Maintain confidentiality of consumer information in compliance with HIPAA regulations.
- Perform additional duties as assigned to support program operations and consumer care.
