Deer Meadows Home Health is seeking a compassionate and skilled Certified Nurse Aide (CNA) to join our Home Health team in Pennsylvania.

Responsibilities

• Provide direct patient care to assigned patients in their homes, including assistance with activities of daily living (ADLs), medication administration, and vital sign monitoring
• Maintain accurate and up-to-date patient records and reports, including vital signs, medication administration, and any notable changes or incidents
• Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions and coordinated care
• Collaborate with other healthcare professionals to develop and implement individualized care plans
• Provide emotional support and companionship to patients, promoting dignity and respect
• Maintain a safe and clean environment for patients, adhering to infection control protocols and policies

Requirements

• Current Certified Nurse Aide (CNA) certification in the state of Pennsylvania
• Must have own reliable transportation; travel reimbursement provided
• Ability to work independently and as part of a team
• Strong communication and interpersonal skills
• Compassionate and patient-focused approach to care
