Job Description

Assist patients via telephone, or other modalities, as part of the primary care team, including call backs to respond to patient needs.

Aid in patient care through the collection of health information, vitals, and other health history information that needs to be shared with the provider and other care team members.

Support healthcare support services for all patients (adult and pediatric) as established by clinic protocols and community practice standards.

Coordinate general patient care needs.

Manage quality initiatives in collaboration with provider and clinical care staff.

Create and support an environment which fosters teamwork, cooperation, respect, and diversity.

Communicate and maintain a professional demeanor with co-workers & patients at all times.

Requirements

  • High School diploma or the equivalent.
  • Currently holds a valid and active CMA certification/registration.
  • 2+ years experience working in a Family/Primary Care setting, Virtual & Telehealth.
  • 2+ years of experience in a Family/Primary Care setting.
  • Excellent communication and teamwork skills.
  • Proven ability to handle calls and address patient needs effectively.

Qualifications

  • High School diploma or the equivalent.
