About the role

Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals.

Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space.

Communicates through various digital channels to support patient care.

Performs initial health screens and intake for new members.

Obtains and reviews old medical records to ensure comprehensive patient history.

Identifies care gaps and orders necessary screening tests.

Confirms appointments and provides instructions to patients.

Conducts pre-visit RAF reviews to prepare for patient visits.

Welcome patients, provides tours of the practice, and reviews welcome packets.

Ensures all medical records are available for office visits.

Performs needed screenings and orders appropriate tests during visits.

Conducts intra-visit RAF reviews to assess patient needs.

Reviews quality metrics, performs vitals, screenings, and medication reconciliation.

Manages the check-out process with patients and conducts post-visit follow-up calls.

Experience with computers required. [Required]

Actively participates in daily team huddles. [Required]

Administer medications, perform vital signs, ECG’s, and various point of care testing per providers orders. [Required]

Other duties as assigned. [Required]

Experience in guest relations or customer service [Preferred]

Qualifications

  • Educated at a Technical/Vocational School
  • Graduate of a Medical Assistant Program
