Certified Home Health Aide (CHHA) : Home Health

About the role

Under the direct supervision of an RN Case Manager, you will provide patient care according to the Plan of Care.

Responsibilities

• Visit patients in their homes to provide care as outlined in the Plan of Care.
• Assess patient needs and document observations accurately.
• Administer medications as prescribed by the physician.
• Provide assistance with daily living activities as needed.
• Communicate effectively with patients, families, and other healthcare providers.
• Ensure compliance with all regulatory standards and policies.

Requirements

• High school diploma or equivalent.
• Current certification as a Certified Home Health Aide (CHHA).
• Valid California Driver’s License.
• Ability to pass a background check and drug screening.

Qualifications

• Experience in a home health setting preferred.
• Strong interpersonal skills and ability to work independently.
• Knowledge of home health care procedures and regulations.

Skills

• Excellent communication and problem-solving skills.
• Physical stamina and ability to perform in physically demanding environments.
• Proficiency in Microsoft Office Suite.

Pay

Starting pay is commensurate with relevant experience above the minimum requirements. The estimated wage range for a Certified Home Health Aide (CHHA) is $55,000.00 - $90,000.00 annually.

Schedule

Full-time positions offer flexible scheduling to accommodate patient needs and personal commitments.
