About the Role

The Certified Diabetes Care and Education Specialist is responsible for the clinical coordination of care related to a specific group of patients or aspect of patient care. Coordination extends across the organization and to the outside community. The role applies specialized knowledge through teaching patients and family, consultation, research, and guidance of other healthcare team members. The position is guided by the organization's Standards of Patient Care and Nursing Practice and demonstrates dedication to professional growth, continuous improvement of patient care, and maintaining specialty certification.

Maintains a minimum of 18 continuing education credits or certificate of attendance hours annually and performs one formal presentation each year.

Responsibilities

• Coordinate clinical care for a specific group of patients or aspect of patient care.
• Provide education to patients and families using specialized diabetes knowledge.
• Consult and guide other members of the healthcare team.
• Engage in research related to diabetes care and education.
• Maintain specialty certification and complete a minimum of 18 continuing education hours annually.
• Deliver one formal presentation per year.
• Work within an ADA Recognized Education Program or other nationally recognized diabetes education program.

Requirements

• Current RN licensure or compact licensure recognized by the State of Vermont.
• ADN required; BSN preferred. The University of Vermont Medical Center is moving toward an all-BSN workforce. External candidates hired into or current employees promoted to an RN position will be required to obtain their BSN as outlined in their offer letter. Current internal RNs are strongly encouraged to pursue a BSN.
• Certified Diabetes Care and Education Specialist certification.
• Minimum of 3 years of relevant clinical experience in diabetes.
• BLS Certification.
