Certified Diabetes Care and Education Specialist

About the role

Develop, implement, and evaluate diabetes education programs within a multi-specialty clinic. Collaborate with healthcare teams to identify patient needs, provide direct education to high-risk diabetic patients, and ensure compliance with care standards. Function as a Case Manager for diabetes patients while promoting professionalism and adherence to regulatory standards.

Requirements

• Active RN license with CDE (Certified Diabetes Educator) certification
• 2+ years of clinical experience in diabetes care or related field
• Strong communication and organizational skills

Preferred Qualifications

• Bachelor’s or Master’s degree in Nursing or related field
• Supervisory or leadership experience
