Certified Clinical Medical Assistant

Position Summary

The Clinical Medical Assistant, under the direct supervision of the Director of Clinical Services, assists in the ongoing support of field clinicians and patients. Builds on the community's resources to plan and direct services to meet the needs of individuals and families within their homes and communities. Conducts required follow-up to ensure initial records meet compliance expectations. Provides clerical support to the hospice team.

Duties & Responsibilities

• Schedule weekly clinical visits with the Director of Clinical Services to facilitate effective patient care and team collaboration.
• Observe, record, and report any changes in patient conditions, including signs and symptoms, the Director of Clinical Services or RN Case Manager as needed.
• Absorb, record, and report any changes in patient conditions, including signs and symptoms, the Director of Clinical Services or RN Case Manager as needed.
• Assist in calling and visiting patients to monitor their health and provide support.
• Answer telephone calls, respond to inquiries, and communicate effectively with staff, patients, and families regarding care plans and services.
• Contact outside physicians listed on the hospice assignments upon patient admission and discharge to ensure continuity of care.
• Order DME (Durable Medical Equipment) based on patient condition and RN requests, ensuring timely delivery and setup of equipment.
• Notify DME companies and pharmacies of patient needs and any changes in care requirements.
