Responsibilities

• Assesses care necessary for client health needs and activities of daily living as directed by the RN case manager.
• Plans and prioritizes client care functions to meet clients' health care needs.
• Implements actions that compliment and support nursing care.
• Evaluates the effectiveness of care provided to ensure quality care and timely completion of client-related activities.

Requirements

• Holds self accountable for delivery of responsible nursing care, demonstrates effective communication skills and interpersonal relationships.
• Assumes responsibility for own continued development and assists with the development of other staff members.

Other Duties and Responsibilities

• Operates equipment as outlined on orientation checklists, policy and procedures.
• Participates in equipment in-services and competencies as they relate to role expectations.
• Performs job skills outlined on orientation checklist as they relate to role expectations.
• Participates in Continuous Quality Improvement as required by the Health System and the job description.

Certifications and Licensures

• BLS - Basic Life Support Required
• FingerPrint - Fingerprinting Required
• PCT - MMH PCT/CNA/EMT/LPN Certification Required
• TEAM ADVANCED - Techniques for Effective Aggression Mgt Required
• Vehicle Insurance Required

Experience, Training and Skills

• Has Ability to read and follow written instructions and document care given.

Physical/Mental Requirements and Typical Working Conditions

• Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.
• Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
• Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.
• Overall vision and hearing is necessary with or without assisted device(s).
• Frequent postural changes such as stooping, kneeling or crouching.
• Some exposure to blood borne pathogens and other potentially infectious material.
• Must follow MyMichigan Health bloodborne pathogen and TB testing as required.
• Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.
• Overall dexterity is required including handling, reaching, grasping, fingering and feeling.
• May require repetition of these movements on a regular to frequent basis.
