About the role

The Center Medical Director at Oak Street Health leads the clinical team in providing exceptional patient care and fostering a great workplace, aiming to keep patients happy, healthy, and out of the hospital. This role balances 80% clinical time practicing primary care alongside provider colleagues and 20% administrative time. The Medical Director holds mission-critical leadership responsibilities, including modeling thoughtful primary care, mentoring and coaching providers, and contributing to organization-wide strategic initiatives.

Responsibilities

  • Support organizational strategy by collaborating with the Practice Manager and Outreach Manager to achieve objectives in care, patient experience, growth, workplace culture, and diversity, equity, and inclusion.
  • Drive implementation of Oak Street Health strategic initiatives and regional priorities under the direction of regional leadership.
  • Cascade organizational information to care teams and escalate team concerns or ideas to leadership.
  • Coordinate and lead the onboarding of new providers at the center.
  • Model excellent medical care, implementation of the care model, and success in metrics.
  • Ensure consistent and robust implementation of the Oak Street Health care model, including daily huddles, patient care planning, admission prevention, and clinical documentation workflows.
  • Guarantee that medical care provided by each provider and care team is evidence-based, keeping patients healthy and out of the hospital.
  • Teach and implement Oak Street's clinical guidelines at the individual provider and center level.
  • Support care teams in delivering high-quality preventative care (primary and secondary).
  • Educate and support providers in implementing a value-based, full-risk model of care.
  • Assess and address root causes when center metric goals are not met.
  • Create and maintain a culture of coaching, growth, and constructive problem-solving for providers and care teams.
  • Conduct performance reviews, discuss provider goals, and develop plans to achieve them.
  • Acknowledge and celebrate care team successes while communicating opportunities for improvement.
  • Support a positive, joyful workplace culture aligned with Oak Street Health values.
  • Perform other duties as assigned.

Requirements

  • Graduation as M.D., D.O., A.P.N., or P.A.
  • Board certification in Internal Medicine or Family Medicine (for M.D. or D.O.) or in Family, Adult, or Gerontology (for A.P.N.).
  • Experience in clinical leadership roles, including leading and coaching providers.
  • Experience using a metrics-driven approach to medical care or quality projects.
  • Passion for work and motivation to contribute to a fast-growing organization.
  • Awareness of leadership skills and potential for impact, with eagerness to share them with a dynamic team.
  • Commitment to serving older adults and low-income patients.
  • Comfort with ambiguity and a love for problem-solving.
  • Ability to remain calm under pressure and guide others toward effective solutions.
  • 5+ years of outpatient practice experience preferred.
  • Fellowship training in Geriatrics, other professional degrees (e.g., M.B.A., J.D., M.P.H.), and prior executive roles welcomed but not required.
  • U.S. work authorization.

Schedule

  • Full-time, 40 hours per week.

Pay

The typical pay range for this role is $174,070.00 - $374,920.00 annually. The actual base salary offer will depend on factors such as experience, education, and geography.
