Key Responsibilities

  • Embraces the goals to enhance the function, mobility, independence, quality of life, and quality of end of life of participants and holds direct reports accountable to support the organization’s goals.
  • Fulfills the regulatory role of the Center Manager, ensuring decisions about medical, social, and supportive services are driven by the IDT decision making process.
  • Hires, trains, evaluates, and disciplines IDT Team Members in coordination with the Chief Operations Officer.
  • Captures and coordinates staff development and training, and assists with other human resource issues in conjunction with GFHS Human Resources Department.
  • Assists with the professional development and supervision of all IDT disciplines, including Home Care Coordinators, therapy service providers, Restorative Aides, Social Workers, Dietitian, transportation representatives, and administrative support staff.
  • Coordinates orientation and in-service programs for staff to meet regulatory requirements and support performance improvement.
  • Takes responsibility for the development, implementation, and execution of participant care.
  • Facilitates activities related to new participants, including new participant orientation.
  • Ensures that participants are oriented to the program and have opportunities to express their opinions about the care and services they receive.
  • Coordinates all IDT Team Meetings, including care planning and implementation, preparation for interdisciplinary conferences, and daily meetings, and family conferences; and all related follow-up.
  • This includes the performance of routine care plan audits and all related follow-up with disciplines’ interventions and evaluations in participants’ care plans.
  • Participates in the continued development of the team to improve the team decision-making process, providing support to the team to troubleshoot, advise, teach, coordinate and deliver care and service to participants.
  • Participates in the Pace of Southern Piedmont Quality Program by collaborating with the Quality Coordinator in support of the Service Delivery and Grievance resolutions and education of said SDAGs to direct reports.
  • Provides participant monthly data to Quality Coordinator for Quality reports.
  • Serves as liaison between individual program disciplines and community service providers.

Qualifications

  • Associates Degree in Health Care Related Field.
  • 5 years of experience in health care management or nursing with at least 2 years supervisory experience.
  • Minimum of 1 year experience working with the frail elderly population.
