CAT Community Action Treatment Case Manager

Chrysalis Health is a leading provider of behavioral healthcare with over 25 years of experience serving communities across Florida. We are seeking a compassionate and driven CAT Case Manager to support youth and families through community-based care and coordination.

About the role

Pay

Up to $45,000 annually, based on experience and performance. Mileage reimbursement and additional incentives may apply.

Responsibilities

  • Meet with youth and families weekly in their home or community setting
  • Assess needs and develop individualized service plans
  • Connect clients to resources including healthcare, education, housing, and employment
  • Coordinate care with families, providers, and community partners
  • Advocate for clients and support long-term stability and independence
  • Assist with transportation to appointments, court, or services when needed
  • Monitor progress and adjust plans to support successful outcomes
  • Complete documentation in a timely and compliant manner

What To Expect

  • Majority of work is in the field (homes, schools, community settings)
  • Driving is required throughout your assigned service areas: Alachua, Baker, Bradford, Gilchrist, Levy, and Union counties
  • Work may include evenings, weekends, and on-call rotation
  • You may support youth and families experiencing behavioral, emotional, or crisis situations

Requirements

  • Bachelor’s degree in Psychology, Social Work, Mental Health Counseling, Marriage & Family Therapy, or related field
  • Minimum 1 year of experience working with youth or behavioral health populations
  • Valid Florida driver’s license, reliable vehicle, and acceptable driving record
  • Ability to pass Level 2 background screening
  • Strong communication, organization, and problem-solving skills
  • Maintain compliance with Medicaid, DCF, and agency standards

Qualifications

Bilingual (Spanish or Creole) is a plus.
