About the role

Act as part of a team responsible for the support, evaluation, and treatment of adults with developmental disabilities living in a residential setting. Perform all case management and operational duties assigned by the Direct Care/Residential Manager.

Responsibilities

  • Provide operational duties assigned by the Residential Manager, including: ordering medication, assisting individuals with banking, monitoring program money and petty cash, grocery shopping, assisting with training new staff.
  • Provide case management duties as needed, including: attending and participating in meetings regarding individual(s), writing monthly progress notes, Individual Support Plans and assessments and objectives for annual reviews, assisting with the development of behavior plans or intervention strategies, collecting data, and completing all other paperwork as requested.
  • Attend weekly staff meetings, document meeting minutes, meet deadlines on all program related paperwork, and act as a team member in the development, implementation, and evaluation of programs and services.
  • Maintain valid Driver’s License and provide transportation as required by program.
  • Responsible for overseeing shift documentation provided by staff to ensure professional and accurate reporting.
  • Instruct, supervise and evaluate individuals in all areas of community living as outlined in each support plan.
  • Record data on all instructional programs.
  • Implement all behavior plan interventions as assigned, which may include physical restraint.
  • Absent from shifts off-site, any hospital stays (i.e., Boston).
  • Administer medications, as required, in accordance with medication administration procedures.
  • Assist and support individuals as needed with scheduling and attending social and leisure activities such as: family gatherings, religious celebrations, community events, and social gatherings.
  • Maintain residence and yard in clean, orderly condition as outlined by Program Manager, including: vacuuming, dusting, cleaning floors, bathrooms, laundry and keeping lawn clear of trash and debris.
  • Actively assist and support individuals as needed with personal care responsibilities, including: diet, exercise, meal preparation, bathing, shaving, dressing, toileting, etc.
  • Assist and support individuals as needed with physical direct care, including: physical or mechanical transfer of individuals from wheelchairs to a van, chair, or shower, etc.
  • Participate in on-the-job training and provide coaching to new co-workers as directed.
  • Adhere to all established lines of communication, dress in a professional manner, and represent a positive and proactive attitude with co-workers, families, and referral agencies.
  • Write reports, including daily shift reports, and other required documentation utilizing professional, and appropriate documentation skills.
  • Attend all required meetings, and adhere to all program routines and the schedules and needs of individuals under supervision.
  • Adhere to agency safety standards.
  • Perform additional duties, or substitute for others as required, including other programs.
  • Perform additional duties, or substitute for others as required. Additional duties may include but are not limited to clearing snow and ice from walkways, vehicles, parking lots, and roads using equipment like shovels and snow blowers while applying deicing agents to ensure safety.
  • Adhere to all RTR policies and procedures.
  • Develop and provide all services in accordance with RTR’s philosophies including staff and Board beliefs.

Qualifications

  • Bachelor’s degree and one year experience in human services or High School Diploma or equivalent with a minimum of two years direct experience in human services.
  • Basic knowledge of Microsoft Word and the internet.
  • Education related to a field such as: psychology, social work, special education or related human service work preferred, and at least one year residential experience.
  • Valid Driver’s License.
  • Successful completion and maintenance of all state and agency mandated trainings, including medication administration.
  • Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.
  • Must be able to perform each essential function satisfactorily.

Physical Demands

  • While performing the duties of this job, the employee is occasionally required to stand, walk, and sit; use fingers, handle, or feel; reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl; and talk and hear.
  • The employee is occasionally required to taste or smell.
  • The employee must frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 60 pounds.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
