Case Worker- Health Educator (Mobile Clinic)

About the Role

Under general supervision, the Case Worker – Health Educator uses advanced skills gained through training and experience to provide specialized educational counseling services within an assigned functional area. This role involves regular communication of program services to clients, following established procedures for routine tasks, and performing varied work with limited decision-making responsibility. Originality and ingenuity are required to apply appropriate procedures and techniques to broad assignments, resolving most questions and problems while serving as a resource for complex issues.

Responsibilities

• Provide advocacy, culturally appropriate health education, and information regarding assigned program objectives to clients and communities.
• Coordinate care, case management, and system navigation activities to ensure access to care.
• Build individual and community capacity by teaching clients how to manage their own health needs.
• Implement individual and community assessments to determine needs and identify risks.
• Conduct outreach, including home visits, to provide education, assessment, and social support.
• Document all services in compliance with the agency’s quality assurance plan.
• Report on program progress at required intervals per funding guidelines.
• Create educational and program marketing materials, such as newsletters, Facebook content, and websites.
• Participate in analytical analysis of the program and identification of participant characteristics.
• Organize activities and/or fundraising events in support of program objectives.
• Maintain agency confidentiality and client privacy.
• Stay current regarding services provided to clients.
• Follow funding sources requirements and CARF standards applicable to the services provided.
• Facilitate delivery of the ACCESS Diabetes Prevention Program, including providing support, services (e.g., taking blood pressure, diabetic foot care), and guidance to participants while maintaining fidelity to the curriculum.
• Act as a cultural bridge between individuals, communities, and health/social service systems.
• Travel to community locations for program delivery and/or deliver programs virtually.
• Attend training events, meetings, and conferences as needed to fulfill funding requirements.
• Translate and interpret information for non-English speaking clients as needed.
• Conduct presentations to audiences of various sizes and demographics.
• Assist and enroll clients in programs, services, and benefits, and refer clients to other services outside the scope of the area.
• Operate standard office equipment and use required software applications.
• Perform other duties and responsibilities as assigned.

Requirements

• Associate degree in Business, Marketing, Public Health, Social Work, or Human Services (or related discipline).
• At least two years of experience in sales, marketing, public health, social work, or human services.
• Experience in group facilitation (in-person and via video conferencing), motivational interviewing, and lifestyle change health coaching is preferred.
• Ability to communicate effectively, both orally and in writing.
• Proficiency in operating standard office equipment and using required software applications, including Microsoft Office.
• Ability to organize and prioritize multiple tasks and meet deadlines.
• Fluency in English; ability to interpret, speak, and write in Arabic, Spanish, or other native languages of the predominant service population is preferred.
• Ability to maintain confidentiality of agency and client information.
• Chauffeur’s License preferred but not required (must be obtained after hire if needed).
• If employed in the diabetes program, certification as a Diabetes Prevention Lifestyle Coach (or obtained within 3 months of hire).

Skills

• Knowledge of community resources, including social services agencies and crisis intervention providers.
• Understanding of processes, rules, and regulations of various services and funding agencies related to eligibility and maintenance of services.
• Basic knowledge of health and wellness, fitness principles, and healthy living behaviors.
• Basic EMR (Electronic Medical Records) functionality.
• Ability to partner with other functional areas to accomplish objectives.

Schedule

Normal business hours, with some additional hours required as needed. Local travel may be required.
