Case Worker- Behavioral Health - SVE

Full-time position located in the Community Health & Research Center.

About the Role

Under general supervision, the Case Worker – Behavioral Health provides a wide array of social work functions, including assessment interviews, client counseling, case evaluation, and other social services to individuals, groups, or families in a behavioral health setting. Responsibilities include providing referrals to community services such as financial assistance or healthcare, interpreting program policies and regulations, and maintaining proper records according to organizational, state, and federal guidelines.

Responsibilities

  • Identify individuals in need and conduct intake processes, including crisis intervention and completion of all required forms.
  • Conduct assessments on ACCESS property, in clients’ homes, schools, hospitals, recreational programs, and/or community agencies.
  • Connect clients to services, support, and resources, including both in-house and external referrals.
  • Explain processes, rules, and regulations regarding service eligibility and maintenance to clients prior to submitting applications.
  • Engage clients in developing individualized goals and advocate on their behalf when interacting with other agencies or companies.
  • Follow up on referred services to ensure client participation and program compliance.
  • Adhere to funding source requirements and CARF standards applicable to the services provided.
  • Document all services in compliance with the agency’s quality assurance plan.
  • Report on program progress at required intervals per funding guidelines.
  • Participate in peer reviews, trainings, and presentations as needed.
  • Maintain agency confidentiality and clients’ privacy.
  • Attend and facilitate client interviews with agencies such as the Department of Health and Human Services or Unemployment, as needed.
  • Develop and implement educational programs targeted toward requested services.
  • Participate in services as part of an interdisciplinary team.
  • Translate and interpret information for non-English speaking clients, if needed.
  • Transport clients on an as-needed basis.
  • Conduct individual or group sessions using specific curricula.
  • Communicate and interact with external program funders and attend grant-related meetings.
  • Participate in grant proposal writing and submissions.
  • Conduct early detection assessments, including family or individual assessments, and create service plans.
  • Participate in program analysis and identification of participant characteristics.
  • Operate standard office equipment and use required software applications.
  • Perform other duties and responsibilities as assigned.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Sociology, or a closely related field.
  • A minimum of two to three years of experience in a closely related position or an equivalent combination of experience, education, and/or training.
  • Valid Driver’s License.
  • Certification as a Qualified Mental Health Professional (QMHP) for Adults (5 hours within 90 days of employment) or QMHP for Children (24 hours within 90 days of employment).

Skills

  • Intermediate to advanced knowledge of crisis intervention techniques.
  • Familiarity with community resources, including social services agencies and crisis intervention providers.
  • Understanding of processes, rules, and regulations of various services and funding agencies.
  • Proficiency in operating standard office equipment and using required software applications, including Microsoft Office, Michigan Single Sign On, and Community Health Automated Processing System (CHAMPS).
  • Ability to actively listen to clients and partner with other functional areas to accomplish objectives.
  • Strong facilitation skills to ensure all viewpoints, ideas, and problems are addressed in meetings.
  • Ability to objectively gather information, identify linkages and trends, and apply findings to assignments.
  • Strong organizational and prioritization skills to meet deadlines.
  • Ability to handle stressful situations and provide a high level of customer service in a calm and professional manner.
  • Excellent oral and written communication skills in English.
  • Ability to interpret, speak, and write in Arabic, Spanish, or other native languages of the predominant service population is preferred.
  • Commitment to maintaining confidentiality of agency and client information.

Schedule

  • Normal business hours, with some additional hours required.
  • May be required to be on-call 24/7 based on assigned functional area.
  • Local and in-state travel may be required.
