Core Responsibilities
- Conducts situational analysis, which may be at home and/or office visits, mentors, and provides technical assistance as needed.
- Collaborates with the service agencies and program leadership to provide and administer services appropriately.
- Develops and provides training to family support networks and/or care providers on various topics related to quality early care and education.
- Maintains and reviews records and files to ensure compliance with project guidelines.
- Collaborates with all funding sources to ensure the availability of adequate resources.
- Maintains current knowledge of services available and changes applicable rules and regulations.
- Attends meetings and training, which may include the Early Learning Coalition, United Way, and the Quality Counts Career Center to enhance developmental potential.
- Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions
- Provide maternal child health services that may include: making initial contacts/phone calls, recruitment, and referral of eligible participants; conducting client assessments; developing individualized care plans to address service needs and risk factors; and providing comprehensive care coordination and wraparound services including parenting education, breastfeeding support, tobacco cessation, inter-conception care education, childbirth education, and psychosocial referral services as needed.
- Assess client needs for housing and vocational training and make referrals as needed; establish partnerships with local housing providers and employers to further facilitate referrals.
- For high-risk clients with substance use or substance exposure risk-factors, develop a plan of care to address risks, assess motivation to engage in treatment, and make treatment referrals.
- Work in conjunction with supervisor and multidisciplinary service team to ensure implementation of coordinated services as related to program participant needs.
- Provide case management/care coordination referrals to participants when appropriate for the range of maternal child health psychosocial and behavioral health needs.
- Perform quality assurance tasks, service-related administrative duties and data tasks related to the preparation of programmatic documentation and agency reports and compliance with program performance measures.
- Conduct programmatic educational outreach activities in community settings and facilitate collaborative relationships with community partners.
- Complete all required client-related service and programmatic written and electronic documentation in compliance with programmatic requirements.
- Attend all required training and staff meetings and strictly adhere to program protocols.
Core Qualifications
- Bachelor’s degree in relevant field required
- Minimum 3 years of relevant experience required
