Case Mgr MWHS - Billingual

About the role

Recruit and maintain a caseload of up to 50 participants, conducting monthly contact with each. Serve 100 participants within a program year. Establish and maintain cooperative, trusting relationships with individuals, families, and clinical/community-based organizations using non-judgmental, caring language.

Responsibilities

• Identify and share Healthy Start information; encourage and navigate referrals to and from clinics and community-based organizations using diverse outreach methods.
• Complete required assessments, including Healthy Start tools, to understand participants, their families, and communities.
• Assist participants/families in creating goals documented on an Individual Family Service Plan, updated quarterly.
• Provide home-based services including health education, breastfeeding/lactation support, doula services, and other case management activities.
• Promote father/partner engagement with the infant/child and discuss the importance of well-child visits, immunizations, and finding a medical home.
• Screen and refer for substance abuse and postpartum depression treatment as indicated.
• Assist participants with completing a Family Reproductive Plan.
• Educate prenatal clients about child development, provide developmental screenings using ASQ SE 2 and ASQ 3 tools, and educate about infant safety, including safe sleep practices.
• Identify children with special needs and make necessary referrals.
• Advocate for and promote culturally and linguistically appropriate services and resources within the Healthy Start program and with community partners.
• Understand and comply with organizational policies, procedures, and Healthy Start guidelines.
• Contribute to prevention and integrated service delivery efforts with other community providers.
• Participate in the agency’s Continuous Quality Improvement Activities.
• Maintain client family records as required by agency and program standards.
• Compile and enter data into ETO as required by the program and agency.
• Provide data and information to assist in program reporting.
• Inform the Healthy Start Team of resources available to community families.
• Develop a consistent schedule for visiting program participants.
• Comply with HIPAA regulations.
• Perform additional duties as assigned.

Requirements

• Bachelor’s degree in Social Work, Early Childhood, or a related field.
• Experience working with mothers, fathers, and families.
• Experience in case management.
• Sensitivity to cultural diversity.
• Ability to communicate and cooperate with diverse families, professionals, and community groups.
• Strong networking and professional partnership development skills.
• Ability to work flexible hours, including occasional evenings and weekends.
• Ability to write routine reports and correspondence.
• Ability to speak effectively before groups of customers, employees, or the organization.
• Exercises discretion and independent judgment in executing tasks.

Skills

• Competent in Microsoft Office tools and inputting services in an online database.
• Bilingual/Bicultural in English and Spanish required.
• Valid Driver’s License and Automobile Insurance.

Physical Requirements

• Must be able to transport oneself to and from home visits.
• Occasionally ascends/descends stairs.
• Constantly positions oneself to assist children during activities.
• Frequently moves desks, chairs, tables, easels, and children's toys.
• At times may be required to lift up to 25 lbs. for various classroom and event needs.
• Frequently moves in outdoor weather conditions.

Pay

$44,000 per year.
