Essential Responsibilities
- Develop, implement, and monitor Individualized Service Plans (ISPs), Rehabilitation Plans, and other person-centered service plans with measurable vocational goals
- Provide ongoing case management, supervision, coaching, and support to assigned participants
- Maintain accurate case notes, documentation, reports, and participant records in accordance with agency and funding source requirements
- Complete monthly, quarterly, and annual reports as required
- Provide individualized job readiness, career exploration, workplace, and life skills training
- Aid participants in developing vocational, community employment, and self-employment opportunities based on their interests, abilities, and goals
- Implement workplace accommodations, job modifications, and individualized supports to promote successful employment outcomes
- Provide crisis intervention, counseling, and problem-solving assistance when necessary
- Serve as a positive role model and advocate while promoting independence, professionalism, and community inclusion
- Conduct and coordinate participant intakes, assessments, evaluations, and eligibility determinations
- Review participant progress and determine continued eligibility for vocational services, coordinating transfers or program transitions when appropriate
- Participate in production floor time studies and quality assurance activities
- Assist with program expansion efforts, including identifying community employment opportunities and supporting grant-related initiatives when assigned
Requirements
- Bachelor's Degree in Human Services, Special Education, Psychology, Social Work, or a related field required
- Minimum of one (1) year of supervisory experience working with individuals with intellectual or developmental disabilities
- Valid New Jersey driver's license in good standing with no more than five (5) points
- Strong leadership, communication, organizational, and case management skills
- Ability to effectively supervise participants while coordinating multiple priorities
- Ability to work independently and collaboratively in both office and community-based settings
