About the Role

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and/or co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Responsibilities

  • Provide direct services and case management to individuals with mental illness and/or co-occurring disorders.
  • Assist clients in living independently within their community.
  • Provide services as a Substance Use Specialist within San Bernardino County FACT, ACT, and MAPS programs.
  • Perform mental health services including interviewing, referring participants, and crisis intervention.
  • Use personal or company vehicle to drive clients to appointments, groups, and meetings.

Requirements

  • High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; OR
  • Associate’s degree and two (2) years of experience; OR
  • Bachelor’s degree in Social Services.
  • At least 1 year of experience providing services as a Substance Use Specialist.
  • For San Bernardino County FACT, ACT, and MAPS programs, in addition to the above, must also have:
    • At least thirty semester (45 quarter) units of completed college coursework in behavioral or social science, or
    • Sixty semester (90 quarter) units of completed college coursework, which includes 15 semester (23 quarter) units in behavioral science (Psych Tech courses and Alcohol and Drug Certificate courses are acceptable).
  • AND one of the following:
    • One (1) year experience providing mental health services including interviewing and referring participants and performing crisis intervention, or
    • Three (3) years of experience in a behavioral health department which involved direct participant contact, or
    • Bachelor’s degree in a behavioral science and six (6) months experience providing mental health services including interviewing and referring participants and performing crisis intervention.
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments, groups, and meetings.
  • Willingness to use the company vehicle to drive clients to appointments and groups.

Pay

Expected starting wage range is $23.00 - $25.15. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

  • Full-Time | DAYS
  • Shifts: 8:00 AM - 4:30 PM
  • Days: Monday - Friday
