Case Manager Substance Use - E6 DTLA
About the role
The Substance Use Case Manager works with the E6 team to support the most vulnerable and chronically homeless individuals in their recovery from substances and in regaining control over their lives. This role is part of a large-scale street-based engagement effort in the Metro Los Angeles Area (Service Area 4) aimed at combating homelessness through a countywide network of multidisciplinary, integrated street-based teams.
Responsibilities
- Utilize harm reduction, social, environmental, and motivational strategies to support clients, collaborating with licensed clinicians, other counselors, case managers, and support staff.
- Encourage and promote a strength-based environment to assist clients in meeting their individual goals.
- Conduct street outreach to identified homeless persons, assisting in engaging them in services and accepting treatment from the program staff.
- Assist with the enrollment process for clients as needed.
- Conduct and document individual counseling sessions with clients at regular intervals determined by the individual's needs.
- Coach clients individually on problems with transitioning from the streets to home, developing supportive independent living skills, maintaining sobriety, and accessing necessary resources in the community.
- Provide referrals to allied critical health living services for clients on caseload.
- Document and account for time spent on client contacts and activities in client records in accordance with agency and funder standards.
- Attend and actively participate in team meetings and program-wide meetings daily.
- Maintain a current, thorough knowledge of ongoing developments and new techniques in the fields of substance abuse, mental health, and behavioral health, utilizing these to provide comprehensive, wrap-around services to clients.
Qualifications
- High School diploma or GED; Associates or Bachelor’s degree in related field preferred.
- Certification in substance abuse counseling (CAADE and/or CAADAC preferred).
- Minimum two years’ experience as a substance abuse counselor and/or case manager, in a licensed and/or accredited substance abuse treatment facility (experience in a dual-diagnosis setting preferred).
- Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers.
- Current, valid California Driver’s license with an acceptable driving record and reliable vehicle.
- Computer literacy, including knowledge of Microsoft Word, Outlook, and Excel.
- Detail-oriented with excellent time management, organizational, written, verbal, and computer skills.
- Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision.
- Able to obtain and maintain CPR/First Aid certification.
- Knowledge of 12-Step organizations, culture, terminology, and literature.
- Working knowledge of criminal justice system.
- Certification as an Anger Management and/or Domestic Violence facilitator.
- Bilingual in English/Spanish (preferred).
- Minimum 30 WPM typing speed.

Work Environment
Combination of field and office environment. On occasion, walk or drive to different local sites. Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 lbs. May necessitate working in busy and loud environments. May be exposed to elements like cold, heat, dust, noise, and odor. May need to bend, stoop, twist, and sit throughout the day.
