What You Will Do To Change Lives

To change lives, you will serve as a primary support and case manager for individuals with mental illness and/or co-occurring disorders. Your responsibilities include:

  • Assisting clients to live as independently as possible in their chosen community.
  • Providing direct services to support clients' recovery and well-being.
  • Collaborating with clients, families, and other healthcare providers to develop and implement individualized treatment plans.
  • Supporting clients in accessing necessary resources and services within the community.
  • Ensuring clients adhere to medication regimens and therapy sessions.
  • Maintaining accurate records and documentation of client progress and interactions.

Shifts Available

  • Full Time | DAYS | Shifts: 8:30 AM - 5:00 PM | Days: Monday - Friday
  • Sign on bonus of $3,000.00
  • Expected starting wage range is $23.00 - $27.63.
  • Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed.
  • Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring To The Table (Must Have)

  • High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; OR Associate’s degree and two (2) years of experience; OR Bachelor’s degree in Social Services.
  • Must possess at least 1 year of experience providing services as a Substance Use Specialist.
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use the company vehicle to drive clients to appointments and groups, etc.
