Key Responsibilities
- Ensure the immediate safety of all individuals being assessed through active crisis management and de-escalation.
- Administer the Columbia Suicide Severity Rating Scale (C-SSRS) to evaluate risk and determine appropriate interventions.
- Develop and implement individualized Wellness Recovery Action Plans (WRAPs) and safety plans.
- Provide gatekeeper training to empower Veterans, family members, and community partners in recognizing warning signs and responding effectively.
Assessment & Individualized Support Planning
- Conduct comprehensive mental health screenings and needs assessments to identify barriers and strengths.
- Develop Individualized Service Plans (ISPs) that promote recovery, self-determination, and personal growth.
- Monitor and document participants' progress, adjusting goals and supports as needed to ensure continued engagement and improvement.
Case Management & Care Coordination
- Refer and connect participants to appropriate community and VA resources, including behavioral health, housing, and employment services.
- Absorb participants with scheduling and attending initial appointments; follow up to confirm participation and continued care.
- Advocate for participants' rights, ensuring they receive equitable, respectful, and trauma-informed services.
Participant Education & Skill Development
- Conduct one-on-one and small-group sessions on essential life and recovery skills, including: Suicide prevention and awareness, Accessing and utilizing community resources, Understanding participant rights, Budgeting and financial literacy, Daily living and coping skills.
Community Outreach & Collaboration
- Maintain and strengthen relationships with existing referral sources, including VA partners, local agencies, and Veteran service organizations.
- Identify and cultivate new referral resources to expand the program's reach and impact.
- Represent the program at community events and collaborate with partners to promote awareness of suicide prevention resources.
Documentation & Compliance
- Maintain accurate, timely, and confidential documentation of all participant interactions in accordance with program and agency standards.
- Ensure compliance with all policies, procedures, and federal regulations governing suicide prevention services.
- Participate in staff meetings, training, and program evaluations to support continuous improvement.
