Case Manager Specialist

As a member of the Assertive Community Treatment (ACT)/Integrated Dual Disorder Treatment (IDDT) team, the Case Manager Specialist ensures continuity of services, focusing on the six case management functions: assessment, treatment, planning, linking, monitoring, and advocacy for consumers with serious mental illness and co-occurring conditions.

Responsibilities

• Complete annual assessment and person-centered planning for consumers requiring the most intensive non-residential services.
• Facilitate treatment plans addressing consumer needs such as housing, relationships, community inclusion, legal, education/employment, healthcare, safety, entitlements, and recovery.
• Determine appropriate level of care using the Level of Care Utilization System (LOCUS) tool.
• Ensure services align with program guidelines and request alternative services as needed.
• Provide crisis intervention, including participation in 24-hour on-call coverage on a rotating basis.
• Serve as a primary SUD therapist for consumers with co-occurring disorders, once credentialed.
• Collaborate with consumers and staff to assess needs, develop treatment goals, and evaluate progress.
• Provide direct clinical care through individual/family counseling, co-occurring services, psychoeducation, and support for daily living and community activities.
• Maintain knowledge of local community resources to advocate for and empower consumers.
• Actively participate in daily team meetings, trainings, and supervision sessions.
• Facilitate transitions of care based on medical necessity and level of care guidelines.
• Assist in quarterly reviews of treatment plan goals.
• Provide or arrange transportation to achieve treatment plan goals.
• Facilitate groups for psychoeducation, community inclusion, and skill-building.

Requirements

• Ability to be credentialed by NLCMHA to provide services in this professional discipline.
• Master’s Degree in Social Work, Counseling, or Psychology preferred; Nursing degree considered.
• Three years of post-degree experience in developmental disabilities or mental illness preferred.
• Experience as a case manager is desirable.
• Experience working with co-occurring consumers is preferable.
• Licensure with the State of Michigan as a licensed professional (e.g., Licensed Masters Social Worker, Licensed Professional Counselor, or Psychologist) preferred; Licensed Bachelors of Social Worker may be considered.
• Certification as a MCBAP Certified Alcohol and Drug Counselor or eligibility for a Developmental Plan through MCBAP to obtain certification.

$71,663 - $81,663

Working Conditions

• May require irregular hours.
• May require travel by personal vehicle if an agency vehicle is unavailable.
• Occasional lifting up to 25 lbs, stretching/reaching, exposure to bloodborne pathogens, infectious disease, hazardous materials, and stooping.
• Frequent hand/finger dexterity, talking/hearing in person and by telephone, and vision for close work.
