About the role
Seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our South Mountain Recovery Health Center.
Responsibilities
- Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals
- Completes assessments, crisis plans and individualized service plans
- Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others
- To ensure that all services prescribed in the individualized service plan are implemented, conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges
- Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure
Requirements
- No experience required with a bachelors degree in behavioral health - we will train you

- Full-Time Employed: 40 hours/week M-F 8:30a - 5:30p
- Location: 3540 E. Baseline Rd. Unit 150, Phoenix, AZ 85042
