About the role
We're dedicated to improving health experiences for everyone. As a Community Care Case Manager at CVS Health, you'll work closely with individuals and families to manage their health needs, connect them with the right resources, and ensure they get the care they need.
Responsibilities
- Collaborate with members/clients, employers, providers, and insurance companies to coordinate care and services.
- Assess and develop plans to improve health outcomes, assist in returning to work, and determine eligibility for benefits.
- Communicate with stakeholders, including medical providers, attorneys, employers, and insurance carriers, to ensure appropriate care.
- Prepare required documentation and interact with internal multidisciplinary teams to enhance health outcomes.
- Provide educational and preventive care information to promote better health.
- Apply relevant laws and regulations, and follow special instructions from insurance carriers and referral sources.
- Conduct evaluations, facilitate integrative functions, and monitor progress towards desired outcomes.
- Develop a proactive approach to address issues and enhance overall wellness.
Requirements
- Reside in the South Region: OH, VA, NC, KY, TN, SC, GA, AL, MS, AR, LA, FLCandidate must have active and unrestricted Compact Registered Nurse (RN) Licensure in OH or in their state of residence.
- 3+ years clinical practical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac.
- 2+ years case management, discharge planning, and/or home health care coordination experience.
Preferred Qualifications
- Bilingual - Spanish
- Excellent analytical and problem-solving skills
- Effective communications, organizational, and interpersonal skills
- Ability to work independently (may require working from home)
- Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint
- Efficient and effective computer skills including navigating multiple systems and keyboarding
- Willing and able to obtain multi-state Registered Nurse (RN) licenses if needed
Education
- Associate's Degree or Nursing Diploma (REQUIRED)
- Bachelor's Degree (PREFERRED)
License
- Active and unrestricted Compact Registered Nurse (RN) Licensure in OH or in their state of residence
