Qualifications
- Persons self-employed or employed as Service Coordinators must be a licensed Master Social Worker, Licensed Clinical Social Worker, or hold a Doctorate or Master of Social Work, Psychology, Gerontology, Physical Therapy, Nursing, Special Education, Rehabilitation Counseling, Speech Pathology, or Occupational Therapy license from the NYS Education Department.
- An individual with a Bachelor’s degree must have at least one year of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.
- An individual with an Associate’s degree must have at least five years of experience providing case management/service coordination, information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.
- Maintain active caseload
- Formally reviewing, updating, and submitting Service Plans to the RRDS for review and ensuring all waiver service providers and the participant receive copies of approved plans
- Conduct monthly face-to-face meetings with assigned participants, including at least one in-home visit no less than once a quarter
- Complete service plans as required by waiver manuals and update them to meet the changing needs of the participant
- Affirm that the PRI/SCREEN is completed at least every twelve months or when the participant experiences a significant improvement in their ability to function independently in the community
- Assist the participant in developing, implementing, and monitoring all services outlined in the service plan
- Maintain records of waiver transportation as described in the Manual section on Transportation Services
- Maintain participant records in accordance with Waiver requirements
- Maintain knowledge of all approved waiver service providers in the region

Responsibilities
Full Time
Full time 40 a week hourly position, travel within the capital district is required.
