About the role
The Case Manager oversees resident services programming at Eden Housing properties serving high needs populations, including seniors, families, individuals with special needs, and those with intellectual disabilities. The role involves coordinating services, developing collaborative relationships, and delivering onsite programs.
Responsibilities
- Deliver onsite programs to meet internal standards and regulatory expectations
- Conduct outreach and marketing to inform residents about available services
- Provide crisis intervention and support for residents with functional impairments
- Support group and one-on-one services, and community building activities
- Develop and implement pilot programs to improve service delivery
- Collect and report data accurately and in a timely manner
- Evaluate program outcomes and contribute to quality improvement
- Engage with local community agencies and service providers
- Support property-level needs assessments and follow-up
- Communicate and execute actions based on resident needs
- Participate in staff meetings and training sessions
Requirements
- Bachelor's or Master's degree in social work, psychology, counseling, or a related field
- 3+ years of experience serving high needs populations, including seniors, families, individuals with special needs, and those with intellectual disabilities
- Experience with assessment tools and service coordination
- Excellent communication and relationship-building skills
- Ability to work independently and manage multiple tasks
- Proficiency in Microsoft Office Suite and other relevant software
