Position: Case Manager
What The Case Manager Does
- Ensure clients are connected to needed services to address immediate needs and maintain permanent housing stability.
- Maintain clear, organized, and audit-ready records.
- Report all services provided to clients into program database monthly.
- Provide all contract funded activities including conducting and completing Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conference, etc.
- Complete all program documentation and enter all client services into Client Track.
- Connect clients to primary medical care and ensure adherence to medical and medication treatment.
- Monitor clients' progress toward their goals via regular contact and document via progress notes.
- Function as client liaison and advocate with outside organizations regarding education, healthcare, housing, legal issues, etc.
- Escort clients to appointments (educational, medical, social service, etc.).
- Aid clients in completing applications for benefits and entitlements and monitor the outcomes.
- Collect required documentation and create client files.
- Conduct at least one home visit per month.
- Report any repair issues to the Housing Specialist.
- Monitor clients' living conditions.
- Meet required monthly projections.
- Provide all required information for the completion of monthly program reports.
- Maintain clients' charts always audit-ready.
- Perform other duties as assigned.
The Case Manager works in support of CAMBA's mission and goals, specifically focusing on the Scattered Site Housing Program (SSHP), which provides 270 scattered site-housing units to HASA clients. The position is located at 19 Winthrop Street, Brooklyn NY 11225.
