Case Manager, Citywide Scattered Site Housing Program

About the role

The Case Manager will be responsible for providing all contract funded activities for a total of 48 units of scattered site housing to chronically homeless individuals with serious and persistent mental illness (SPMI). The apartments are located throughout New York City. Case management staff focus on removing barriers to maintain housing by ensuring that all clients' basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. The operations component focuses on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases, and making units ready when there is a vacancy. Clients contribute 30% of their income towards their rent.

Responsibilities

  • Conducting and completing Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conference, etc.
  • Completing all program documentation and entering all client services into Client Track
  • Ensuring clients are connected to and maintain primary medical care and adhere to medical and medication treatment
  • Connecting clients to needed services, such as medical, mental health, substance use treatment, as well as legal, entitlements, etc.
  • Monitoring clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits, and documenting via progress notes
  • Functioning as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.
  • Escorting clients to appointments (educational, medical, social service, etc.)
  • Assisting clients in completing applications for benefits and entitlements and monitoring outcome of process
  • Collecting all required documentation and creating client files
  • Conducting a minimum of one home visit per month
  • Reporting any repair issues to the Housing Specialist
  • Monitoring clients' living conditions
  • Meeting required monthly projections
  • Providing all required information for the completion of monthly program reports
  • Maintaining clients' charts always audit ready

Qualifications

  • Bachelor or Associate degree with one year of equivalent experience, or
  • Two years of equivalent experience with sufficient education and technical expertise to comprehend written and oral instructions (work orders), basic math to accomplish maintenance tasks, and to document accomplished tasks
  • Relevant experience, preferably in a mid-sized (250+ employees) organization working with persons with histories of homelessness, mental illness, and/or substance abuse
  • Good written & verbal communication
  • Computer literacy in Microsoft Office Suite
  • Bi-lingual English and Spanish

Compensation

$51,500 annually. When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
