We are currently seeking a compassionate and motivated RN Case Manager to join our Colorado team.
Responsibilities
- Conduct in-home clinical assessments and help patients and caregivers successfully begin services.
- Develop, implement, and update individualized plans of care based on each patient's needs, goals, and clinical condition.
- Build ongoing relationships with patients and caregivers through scheduled home visits and regular clinical follow-up.
- Use clinical judgment to identify changes in condition, safety concerns, and barriers to successful care.
- Coordinate care with physicians, caregivers, internal clinical teams, and community resources.
- Educate and coach patients and caregivers on care plans, best practices, health and safety needs, and available resources.
- Review caregiver documentation and respond appropriately to clinical changes or concerns.
- Advocate for patients and help ensure they receive safe, effective, person-centered care.
- Maintain accurate clinical documentation and ensure care is delivered in accordance with applicable regulatory requirements and company standards.
- Collaborate with a supportive interdisciplinary team to improve patient outcomes and the overall care experience.
Requirements
- Graduate of an accredited nursing program.
- Active and unrestricted Registered Nurse license in the State of Colorado.
- Strong clinical assessment, communication, and documentation skills.
- A compassionate, patient-centered approach to care.
- The ability to communicate effectively with patients, families, caregivers, physicians, and other members of the care team.
- Comfort working independently while remaining closely connected to a collaborative clinical team.
- Ability to travel by car within an assigned geographic area.
- Valid driver's license and active auto insurance.
- Comfort using common clinical, case management, and productivity software.

- Proficiency in Microsoft Office Suite.
- Experience with electronic health records (EHRs).

Pay
$90,000—$100,000 USD per year at the commencement of your first year of employment.
Schedule
This position requires travel by car within an assigned geographic area.
