What You’ll Do
- Cook up ongoing assessments of patient clinical status and level of care
- Evaluate and tweak care plans to ensure realistic, goal-driven outcomes
- Collaborate with patients, families, interdisciplinary teams, and payors
- Manage resource utilization including services, consults, and length of stay
- Coordinate all aspects of discharge planning process and transitions of care
- Track and analyze outcomes data to improve care delivery
- Serve as a resource for staff on managed care and regulatory guidelines
- Maintain accurate, timely, and compliant documentation
What We’re Looking For
- An active Massachusetts LPN, RN, or Social Work license
- One to two years of experience in case management, skilled nursing, sub-acute care, discharge planning, managed care, or Medicare
- A solid grasp of managed care and reimbursement
- Excellent communication and critical thinking skills
- The ability to work independently and collaboratively
