Case Manager - Residential Center for Healing & Resilience

Successful candidates will have a once-in-a-career opportunity to be a part of the opening of our new Residential Center for Healing and Resilience, a 64-bed psychiatric residential treatment facility (PRTF) for youth, at our beautiful 258-acre Hands of Promise Campus in Idaho’s Treasure Valley.
As a Case Manager, you will provide case management functions and duties for youth and families receiving Idaho Youth Ranch services. This is a collaborative position where you will work with the clinical and medical team at the RCHR, client family, and referral sources to meet the comprehensive needs of the client.
Responsibilities
- Provide case management for youth receiving inpatient residential care.
- Assess, plan, link, coordinate, and monitor options and services that address a youth and family’s needs.
- Help clients learn, gain, and maintain access to services and providers.
- Provide skill-building services specific to goals identified in the individualized treatment plan to the point where the client may be safely, efficiently, and effectively treated in the least restrictive service level.
Requirements
- Social Work Licensure or Bachelor’s degree in behavioral science or related field with 1 year of experience with children in a social service setting or 5 years of experience with children in a social service setting.
- Ability to complete annual certification of CANS (Child and Adolescent Needs and Strengths).
- Demonstrate knowledge of Medicaid’s Youth Empowerment Services Program.
- Demonstrate a working understanding of HIPAA and Medicaid guidelines and ability to maintain youth confidentiality and privacy.
