Case Manager, Rehabilitation (CM), PRN

About the role

As a Case Manager joining our team, you're embracing a vital mission dedicated to making communities healthier. This is a PRN (as-needed) position.

Responsibilities

• Complete departmental orientation as well as initial and annual competencies.
• Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge.
• Complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedule and conduct family conferences and caregiver communications following team meetings or as needed.
• Facilitate weekly care team conferences to review patient progress and adjust treatment plans.
• Review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions; notify the Health Information Management (HIM) team as needed.
• Perform other duties as assigned.

Requirements

• Bachelor's degree from an accredited nursing school or Social Work program.
• Basic Life Support certification is required within 30 days of hire.
• ACLS and PALS are required within six months of hire.
• One year of experience preferred.
• ASLS certification preferred.

Pay

Hourly range: $45-$55 per hour.
